# Gout symptoms: what to notice and when to get help

> General educational information, not medical advice, diagnosis, or treatment.

Gout often starts suddenly, sometimes overnight, with severe pain, swelling, warmth, and a joint that is extremely tender to touch. The base of the big toe is a common site, but it is not the only one. Symptoms can suggest gout; they cannot confirm it at home. [S03] [S08]

## What a flare can feel like

Using the joint may become difficult. Even light contact can hurt. Visible redness varies with skin tone, so pay attention to warmth, swelling, pain, and changes in function as well as color. [S03] [S08]

Ankles, knees, midfoot, hands, wrists, and elbows can also be affected. Some people have several painful joints or more persistent symptoms. Neither a familiar location nor an unusual one settles the diagnosis. [S03] [S06]

## When to seek assessment

Have a first episode evaluated. If you already have gout, seek review when symptoms differ from your usual pattern, worsen quickly, or do not improve with your prescribed plan. Fever, chills, or feeling unwell with a swollen joint need urgent attention. Gout and joint infection can occur together, and infection can occur without fever. [S03] [S05] [S08]

## Make the history easier to explain

Write down when symptoms started, how fast they worsened, which joints are involved, and whether you can walk or use the affected limb. Mention recent illness, wounds, procedures, injections, or injury. Bring your medicine list and any previous test results. [S03]

You do not need a perfect description. “It started overnight, and I can’t put weight on it” is useful information.

## Where to go next

Read [urgent-care guidance](https://www.gout-life.com/symptoms/when-to-get-urgent-help/) first if you are concerned about infection or severe illness. For a new episode, [preparing for a first assessment](https://www.gout-life.com/symptoms/first-possible-flare/) explains what to bring. If the diagnosis is uncertain, see [how gout is diagnosed](https://www.gout-life.com/symptoms/how-gout-is-diagnosed/).
