# When gout needs specialist treatment

Specialist review can help when the diagnosis is uncertain or standard treatment is ineffective, intolerable, or contraindicated. Complex kidney disease and transplant care also deserve tailored input. Before calling gout refractory, review urate targets, treatment access and use, and whether the diagnosis still fits. [S03]

## Pegloticase

Pegloticase is an infused treatment for chronic gout refractory to conventional treatment. The US label recommends co-treatment with weekly methotrexate and folate supplementation where appropriate; treatment without methotrexate may be used when it is unsuitable or contraindicated. Both therapies need specialist assessment. [S13]

Serious allergic and infusion reactions can occur. G6PD deficiency is a contraindication because of blood-related risks, and at-risk people need screening. Urate is checked before infusions because loss of response can signal increased reaction risk. The specialist also directs stopping oral urate-lowering treatment before this regimen. Do not make that change independently. [S13]

## Medicines targeting IL-1 inflammation

Canakinumab has a US indication for selected adult flares when NSAIDs and colchicine are unsuitable or insufficient and repeated steroid courses are inappropriate. Infection precautions and retreatment restrictions matter. It treats inflammation rather than lowering urate production. Anakinra is used in some specialist settings, but does not have the same US gout indication. [S14]

## Tophi, skin, and movement

Large deposits can affect mobility, skin, and daily activities. Urate control remains central; selected complications may also need orthopedic or wound assessment. Before-and-after images show particular cases and cannot promise the same outcome for everyone. [S03] [S13]

## Prepare for a referral

Bring previous urate results, treatment doses and dates, side effects, flare history, and the practical barriers you have faced. Ask what the proposed treatment is intended to achieve, what monitoring it requires, and how you will judge progress together.
