Gout medicines: interactions and safety
Prepared 2026-10-29 · Read as Markdown
An interaction check needs the whole medicine list: prescriptions, over-the-counter pain relief, vitamins, herbs, homeopathic products, recent antibiotics, and occasional treatments. Exact products and doses, kidney function, and liver function all matter. This page highlights questions to raise; it cannot clear a combination as safe for you. [S09] [S10] [S20]
Important examples to have checked
| Medicine | Why a review matters |
|---|---|
| Allopurinol | Azathioprine and mercaptopurine need specialist handling; other drugs can affect rash or toxicity risk. |
| Febuxostat | US labeling contraindicates use with azathioprine or mercaptopurine. |
| Colchicine | Certain CYP3A4 and P-glycoprotein inhibitors can cause dangerous accumulation, especially with kidney or liver impairment. |
| Probenecid | It changes clearance of several drugs, including methotrexate; salicylates can oppose its urate-removing effect. |
| Pegloticase | The specialist regimen includes specific directions for oral ULT and methotrexate co-treatment. |
These examples are not a complete interaction database. S09 S10 S11 S12 S13
Keep the reason for other medicines in view
Diuretics can affect urate, but may be treating an important condition. ACR conditionally supports switching hydrochlorothiazide or choosing losartan when feasible, while advising against stopping indicated low-dose aspirin. Do not discontinue heart or transplant medicines because they appear on a gout-risk list. S02
Bring the supplement container
Natural products can contain active substances, interact with prescriptions, and vary in ingredients. A package or exact ingredient list helps a pharmacist assess what you are actually using. S20
Emergencies need care, not another search
A new rash while taking allopurinol means stopping it and seeking prompt medical advice. Severe skin symptoms, breathing difficulty, swelling, or severe illness need emergency assessment. A possible colchicine overdose needs immediate help even before symptoms develop. S09 S10
Have new prescriptions and changes checked rather than assuming a previous interaction review still covers them.
