Gout diagnosis: clinical reference and differential

Prepared 2026-10-29 · Read as Markdown

In acute painful, swollen arthritis, assess septic arthritis, CPPD, and other inflammatory arthritis. Suspected septic arthritis requires immediate referral through the local pathway. Gout can coexist with infection; a history of crystals or imaging deposits does not resolve the current infection question. [S03] [S05]

Blood-sample tube, joint-fluid container and microscope, and blank-screen ultrasound equipment illustrate different test questions, with a clinical-interpretation reminder.

Serum urate in context

NICE uses serum urate of at least 360 µmol/L (6 mg/dL) with symptoms and signs to support the clinical diagnosis. When a lower value during a flare conflicts with strong suspicion, repeat at least two weeks after resolution. Hyperuricemia alone is insufficient, and asymptomatic high urate is distinct from established gout. S02 S03 S04

Aspiration and sample interpretation

Consider aspiration and crystal microscopy when diagnosis remains uncertain or unconfirmed. Identifying monosodium urate crystals establishes deposition. Arrange appropriate microbiological assessment where infection is relevant. Sampling, handling, and microscopy expertise matter: an unsuccessful aspiration or negative crystal finding is not a universal exclusion rule. S03 S05

Imaging

EULAR recommends ultrasound and DECT for gout diagnostic assessment. Characteristic findings support diagnosis in context; ultrasound also evaluates inflammation, while DECT is particularly useful for material characterization. Asymptomatic deposition, artifacts, early-disease sensitivity, and incomplete specificity require expert interpretation. Aspiration may still be necessary to assess possible infection. S05

Document the unresolved questions

Record the basis of diagnosis, differential assessment, deposition evidence, infection assessment, flare context of urate sampling, and follow-up for uncertainty. Distinguish research classification from clinical diagnosis. No consumer symptom score in this project is validated as a substitute.
