Febuxostat: its role and cardiovascular warning
Prepared 2026-10-29 · Read as Markdown
Febuxostat lowers urate production and is used for long-term gout control, rather than immediate flare pain. Its place in treatment differs by jurisdiction. US labeling reserves it for people whose adequately titrated allopurinol is ineffective, intolerable, or inadvisable. NICE allows either medicine initially but favors allopurinol with major cardiovascular disease. [S03] [S11]
Understand the US boxed warning
The US label warns about cardiovascular death based on a trial in people with gout and established cardiovascular disease. Discuss your history, alternatives, and treatment goals with the prescriber. Do not stop your medicine solely because of a website summary. Chest pain, severe breathlessness, or stroke symptoms require emergency care. S11
Why you may hear different things about the trials
CARES and FAST had different populations and designs and produced different findings. FAST involved older people already receiving allopurinol with an additional cardiovascular risk factor and reported non-inferiority for its cardiovascular outcome. It does not remove the US warning or show identical safety in every situation. Numerical comparisons need a fuller examination of methods and limitations than a headline provides. S11 S22
Questions before starting or changing treatment
Ask whether allopurinol was tried at an appropriate adjusted dose, how your heart history affects the choice, and what kidney and liver results mean for monitoring. Ask about early-flare prevention and have all medicines checked. US labeling contraindicates febuxostat with azathioprine or mercaptopurine. S11
Follow the target and safety plan
Repeat blood results help guide treatment. Flares can occur as urate changes, so an attack alone cannot tell you what the dose should be. Persistent symptoms need review of urate, preventive treatment, medicine use, and diagnostic confidence. S02
The aim is a decision you understand, with clear reasons for the chosen medicine and clear instructions about follow-up.
