A flare and urate record for your next appointment

Prepared 2026-10-29 · Read as Markdown

A short record can help explain what has happened between appointments. You can copy the fields below into your own notebook or document. The browser-based tracker provides an optional device-saved record without an account.

Record observations without assuming every pain is gout or that a meal caused an attack. A clinician-confirmed flare, a possible flare, and an episode of uncertain cause should remain distinguishable.

Keep a flare and urate record

Use this to prepare for appointments. It does not diagnose an episode, identify a food cause, or recommend dose changes.

Without this choice, entries stay only in this open page and disappear when it reloads. Device storage is not a backup and may be visible to others using this browser.

Downloaded files contain your health notes. Save or share them only where you intend. No entries are uploaded or emailed. Keep original units: this record does not silently convert or compare them.

Separate cards distinguish a laboratory reference interval from a gout treatment target, with a reminder to keep blood urate values and mg/dL or µmol/L units together.

Keep the actual laboratory report if you can. Do not mix units or assume the lab reference range is your treatment target. S02 S03

Use the record to ask better questions

Early attacks can occur during ULT initiation. One result or episode cannot establish treatment failure or tell you how to change a dose. Persistent symptoms, adverse effects, or diagnostic uncertainty need review. A diary can support that conversation without replacing it. S02 S03

Do not wait to record an emergency

If symptoms suggest urgent illness or a serious reaction, seek care first. Read when to get urgent help.
